Beneficiary Designation

The Fire and Police Employees’ Retirement System (F&P) has two forms for the purpose of designating a beneficiary for death benefits payable on account of death prior to retirement; the Standard Beneficiary Designation Form and the Specific Beneficiary Designation Form. Each form offers its own advantages and disadvantages.

The Standard Beneficiary Designation Form

  • · Automatically appoints surviving next of kin in a specific order
  • · Changes your beneficiary as your marital status or survivors change
  • · Members may not alter specific order

The Specific Beneficiary Designation Form

  • · You can name anyone you want as beneficiary
  • · You can name primary and contingent beneficiaries
  • · A new form must be completed each time you want to change your beneficiary

The F&P will only accept one form therefore you must select which form, the standard beneficiary designation form or the specific beneficiary designation form works best for you then complete and submit only that form.

All beneficiary designation forms, whether you select the standard beneficiary designation form or the specific beneficiary designation form, must be properly completed and notarized to be accepted by the F&P.  The F&P will not accept an incomplete beneficiary designation form. The F&P has notaries public who will notarize your form free of charge when you come into the office to complete the form. Forms that are incomplete, not signed, dated, or notarized will be returned to you and this delay could be costly to both you and your beneficiary.

Members who name a minor child or children as beneficiary (ies) must also complete a designation of custodian for a minor child form for each child.

Complete a designation of custodian for a minor child form for each minor child regardless of   whether you have completed a standard beneficiary designation form or a specific beneficiary designation form.

The standard beneficiary designation form, the specific beneficiary designation form, and the designation of custodian for a minor child form are available 24 hours a day on the F&P website www.bcfpers.org. Click on the Active Members then proceed to Forms.
